All athletes must have a current physical on file to participate in athletic activities. The NCHSAA physical form must be completed and signed by the parent, student and doctor who performed the physical prior to participation in any tryouts,practices, or games. Physicals are good for 1 year from the date listed on the physical form. Completed physical forms are on file in Coach Boltinhouse's room at the school. Physical forms maybe obtained in the front office at the school.
Note: A local health care provider usually gives physicals at school during the month of May for a very reasonable fee. This physical will be current the entire next school year.

SCHS/Gfeller-Waller Concussion Awareness Act Compliance.
Concussion awareness forms shall be completed by all coaches, parents, and athletes prior to trying out for any sport. Additionally, a face to face briefing shall be attended by the above prior to the start of the sport season. These forms remain on file and must be completed each school year. Any injury resulting in concussion symptoms must be reported to coaches and sports medicine. If symptoms indicate possible concussion the athlete must be removed from play until cleared by a Licensed Physician including M.D. or D.O. Clearance must be on the NCHSAA Return to Play Form. Sports Medicine is responsible for ensuring the proper documentation and administration of this form. Historically there is a 6 day gradual return to play period following clearance from the doctor.

All players and cheerleaders shall ride the activity bus to and from games; players should go and come as a team. ATHLETES MAY BE RELEASED TO RIDE HOME ONLY TO THEIR PARENTS AT AWAY GAMES. A student must be enrolled in a minimum load of courses at Spring Creek High School. A minimum load of courses as defined by the NCHSAA is 3 courses on the block schedule. The Student must have passed at least 3 out of 4 classes (high school) or 5 out of seven core subjects (middle school) per semester. The student must have been in attendance for 85% of the previous semester (cannot miss more than 13.5 days per semester). The student must not have ever been convicted of a felony. The student must have a completed, up to date physical form and athletic participation form on file before they will be allowed to tryout. Physicals are good for one calendar year. Must attend the school in the district he/she has been assigned. Out of district students (high school ) must attend for an entire year before they are eligible for athletics. The student must not have exceeded eight consecutive semesters since entering the ninth grade. The student must be less than 15 years of age on or before October 16, of the current school year for middle school. the student must be less than 19 years of age on or before October 16, of the current school year for high school. Spring Creek does not tolerate ANY form of bullying and/or hazing at ANY TIME to include, but not limited to, athletic tryout, practice, transportation, game, or during the school day.
