Immunization Requirements - 2022-2023

WAYNE COUNTY PUBLIC SCHOOLS

GRADE SPECIFIC IMMUNIZATION REQUIREMENTS

(for use by school personnel)

      

GRADES K THROUGH 12TH

School Year 2022-2023

 

Students entering school for the 2022-2023 school year are required to have:

 

5 DTP/DTaP (Diphtheria, Tetanus, Pertussis): One dose must have been given on or after the 4th birthday. 4 doses are acceptable if 4th dose was given on/after 4th birthday.                   

 

4 OPV/IPV (Polio): 4th dose is required to be given after the 4th birthday.

                        If the 3rd dose is given after the 4th birthday, a 4th dose is not required.

 

3 Hepatitis B (Hepatitis B):  As of March 2005, the last dose of Hepatitis B vaccine series shall not be administered prior to 24 weeks of age.

 

2 MMR *(Measles, Mumps and Rubella): The first measles or MMR must be given on/after the first birthday AND the 2 doses must be at least 30 days apart. Measles, Mumps, and Rubella are usually given together. If given separately, there should be 2 Measles, 1 Mumps, and 1 Rubella.

 

1 Hib  (Haemophilus Influenzae type B): Only for students under age 5.  Once a student turns 5, HIB vaccine is no longer recommended.

 

4 PCV *(Pneumococcal Conjugate Vaccine): Four doses by 15 monthsAt 5 years of age, PCV is no longer a requirement.

 

2 Varicella: Administered at least 28 days apart for students entering kindergarten.

*Exception: documentation of varicella disease.

 

Tdap (Tetanus, Diphtheria, Pertussis) Booster Dose: All students entering 7th grade or are 12 years of age (whichever comes first) will be required to have a booster dose of Tdap (Tetanus, Diphtheria, Pertussis) if five years or more have passed since the last dose of tetanus/diphtheria toxoid.

 

Meningococcal Conjugate Vaccine (MCV): Two doses. One dose is required for individuals entering the 7th grade or by 12 years of age, whichever comes first, on or after July 1, 2015. 

A booster dose is required for individuals entering the 12th grade or by 17 years of age, whichever comes first. The booster dose does not apply to individuals who entered the 12th grade before August 1, 2020. If the first dose is administered on or after the 16th birthday, a booster dose is not required. Individuals born before January 1, 2003, shall not be required to receive meningococcal conjugate vaccine.

NO STUDENT MAY ATTEND ANY GRADE (K-12) OF ANY SCHOOL WITHOUT PRESENTING A CERTIFICATE OF IMMUNIZATION.  If a certificate is not presented on the student’s first day of attendance, notice in writing, must be given to the parent.  The parent has only 30 calendar days from the student’s first day of attendance to show proof of the required immunizations or a legitimate exemption statement. *Exception for homeless or migrant students.

Without this proof, principals must exclude students from school until this requirement is met.   (An appointment alone is not sufficient.)
