New Student Registration

District enrollment information can be located on the District Enrollment Services page. It explains in detail the required process of enrolling students. 

The following items are required for registration of a new student at Northwest Elementary School:

  • Shot Record

  • Certified Copy of Birth Certificate

  • Proof of Residence - Please provide two verifications from the items listed below:

    • Current driver’s license

    • Current automobile registration

    • Current utility bill

    • Current cable/satellite/internet bill

    • Current telephone bill

    • Property/tax records

    • Mortgage document or copy of deed

    • Residential lease

    • Voter Registration form

    • Military ID card

    • Motor vehicle registration

    • REAL ID

    • US Passport

    • Bank Statement

    • Mediaid card

    • Payroll/W-2 stub

    • Correspondence from government authority regarding receipt of benefits 



* All documents must show the name of the parent or legal guardian and the street or road address of the residence and be current within one month.
