PARENT DAILY SYMPTOM SCREENING CHECKLIST
Parents: Current guidance from the North Carolina Department of Health and Human Services and the National Center for Disease Control recommend that schools no longer screen upon entry at school each day. Instead, NCDHSS and CDC recommend that the parent/guardian screen their child(ren) daily, as listed below, and keep a student home from school according to these guidelines.
If a child has any of the symptoms listed in #3 below that occur while they are at school, you will be contacted and your child will be directed to stay home from school for 10 days, unless COVID is ruled out by a COVID PCR test or by receiving an alternate diagnosis from a health care provider.
1- Since your child was last at school, has he/she been diagnosed with COVID-19?
-If Yes student must stay home 10 days from diagnosis.
2- Has your child had close contact (within 6 feet for 15 minutes or more) with someone diagnosed with COVID-19, or has any health provider contacted you and advised you to quarantine?
-If Yes student must stay home 10 days from exposure.
3- Does your child have any of these symptoms?
-Fever of 100.4 or higher
-New loss of taste or smell
-Diarrhea or vomiting
-New onset of severe headache
-If Yes to any of the symptoms listed in #3, student must stay home 10 days from symptom onset or, until an alternate diagnosis is received from your child’s health care provider, or your child receives a negative COVID PCR test.

If COVID is ruled out by the COVID test or by a physician your child should stay home until 24 hours after symptoms resolve.
