NEW ACCOUNT SIGN UP DIRECTIONS

Every athlete MUST be registered in Final Forms in order to participate in athletics at Charles B Aycock High School.

Please follow this link https://waynecounty-nc.finalforms.com/ to set up your parent and player accounts and to register for all sports played at Charles B. Aycock High School.  These accounts and all paperwork on the site, MUST be electronically signed by the parent and the player BEFORE the first day of tryouts.

Coaches and Administrators will communicate all announcements about tryouts, practices, games, and important events through Final Forms.  Additional information like game schedules can be found on the CBA school website under “Athletics” or on the main page calendar of events.

Quick Guide:

Steps to sign up.

Go to the above website and click on “New Account” under PARENT.

  1.  Parents MUST create and set up their accounts first before a student can create their account.  Parents only need to set up 1 account for the whole family. Siblings who attend CBA in the future (or current NOR students) can just be added to the original parent account. Parents: Be sure the email address you use is one that you check regularly.

  2. Students must then click on the STUDENT tab on the sign in page and create your account. Students MUST use their WCPS email address to set up their accounts. The use of non-WCPS email accounts can cause issues with communication and the completion of forms.

  3. After both accounts have been set up, please click on the blue FORMS button on the upper right corner of the page. Be sure that parents AND players have signed at the bottom of ALL pages to be “in the GREEN”.

  4. Sports physicals:  Sports physicals can be uploaded by the parent by clicking on the folder ICON and uploading either a .jpg or a .pdf file of the physical.  Players can also bring a paper copy of their current physical and we will upload it at the school and return the original to the student.
